We will be asking about how your household uses the Internet and how you see the benefits of broadband (high-speed Internet access). Your feedback is important in order to understand the impact of broadband for your community and to measure its impact to the local economy.

Please note that your privacy will be protected and the information gathered through this survey will only be used for reporting of combined results. No individual responses from any individual, or organization, will be shared with any third party.

The focus of this questionnaire is on the uses and benefits for you and your household from having Internet connectivity, whether you use dial-up Internet or one of various forms of high-speed broadband connections (including mobile wireless devices). Broadband refers to any form of high-speed Internet access at download speeds of 768 KB per second or faster.

Every business and organization completing the survey will have the option to receive a peer group report on Internet usage in Kentucky.
